Go to www.ssa.gov. You will find a directory of Social Security Offices and if you type in your address it will show you the nearest one. Then find form SSA-SS5. Complete and be sure to read about the identification you will need. Social Security is very careful about identification. Bring the form and your ID to the office. Your new card will be mailed to you but you can ask for a print out of your number which you can immediately use as a card.
