Just tell them you have this massive crippling anxiety disorder which has a strong chance of making you an unreliable juror. If it causes you to have to pull out that could mean the huge expense of starting the trial all over again. So you could ask them to exempt you on medical grounds. If they refuse have your condition noted anyway, then if you are made to go & if you get really badly ill that's their fault & they're responsible. A doctor's note would be very helpful to back up your claim.
